We have learned a great deal through our joint collaboration and also from our truly outstanding contributing authors. We would like to thank them all most sincerely for their excellent work and the intellectual stimulation they provided. In many cases, we have been delighted to discover a new approach or modification to the common problems we cover in this book. Shyamala would like to acknowledge the extreme patience shown by her husband and family for more than a year as she deserted them most weekends, writing and editing the cases and finding photos for case series. Shyamala would like to thank her longstanding Mohs technician, Jodie Wegener, for her help with filing clinical images. Duncan would like to again acknowledge the forbearance of his family for this additional project. He knew Shyamala was the right person to lead this endeavour and he would like to thank her for her dedication, high standards and her friendship.
